Default Prepping for ENT appointment?

It's been years since I've seen an ENT or an audiologist. My insurance won't cover an audiology appointment unless I see an ENT first who writes a referral for it. So, to that end, I'm going to see an ENT on July 10th.

What questions should I expect? How can I best prepare for the appointment? Would I be best served by taking in a list of all the problems I've noticed with my hearing, possibly separated by what has existed for a while and what's new? I just don't want to miss anything.

They will probably send you forms to fill out.. where you list your medical history and current problems. It a pain trying to fill them out while waiting in the lobby so I would call and ask them to send them to you ahead of time most do as SOP.

Ditto to filling out the paperwork ahead of time! Definitely list your long-term and short-term problems, in the order that they most bother/hinder you. This will help the ENT/Audi know what to focus on first. Also write down your questions ahead of time so you don't forget when you're seeing him, I always forget in there!
